How to register

We have an open list and welcome requests for registration from patients living in or moving to the practice area. Please check that you are living at an address within our catchment area .

You can register by any of the following methods, collecting registration forms from reception, calling and copies will be posted to you or by downloading.

NHS Form GMS1 – Family Doctor Service Registration – one per each member of your household.

New patient Questionnaire form – One per adult

Health Visitor form – to be completed for All children aged 5 and under.

There are two matters relating to your records that you need to review:-

Summary Care Records (SCR)- If you do nothing, you are automatically opt-in. Please complete the relevant for if you wish to opt-out. We encourage you to remain ‘opt-in’

GP Online – We strongly encourage all patients with online access to sign up for this service. Online application form

Once you have completed please return to the practice.

Please note that online services can only be processed upon receipt of 2 forms of ID, one with a photograph of yourself and one with proof of your home address. (Document date must be within last 3 months)

We recommend that new patients under take a health check with a doctor or a nurse depending on your medication.

NHS staff, patients and visitors must continue to wear face coverings in healthcare settings, unless they are exempt.
