Village Agent

CAT SWEATMAN  – VILLAGE AGENT

My role as Village Agents is to bridge the gap between isolated, excluded, vulnerable and lonely individuals and statutory and/or voluntary organisations which offer specific solutions to identified needs. All of the support that I offer is free and confidential.

Do you live alone, have a problem and are not sure who to turn to?

Do you need some help  finding the right agency, service or people to turn to?

Are you a carer that needs help and support?

Does your community have a collective problem that needs a kick-start?

Or, if you’re just finding life a bit hard going at the moment, I may be able to help.

I am offering clinics at Lister House Surgery on a Monday and Friday as well as at the Community Rooms, Milverton on Thursday afternoons 12 – 3 every week. Here you will able to book an appointment to see me or pop in for a chat if you are passing.

If you would prefer to meet with me elsewhere, I run a Talking Cafe at The Bear Inn, North Street, Wiveliscombe on a Thursday morning between 10am and 12 noon and can be found at Wiveliscombe House in The Square, Wiveliscombe on a Tuesday morning between 10am and 1pm. I am also able to make home visits if this suits your needs better.

I work 5 days a week and will answer my mobile if I am able to. If I don’t answer, please do leave me a message as I will call you back as soon as I can. Any query, large or small – get in touch.

All GP practices are affected by the current National shortage of blood bottles and are taking urgent steps with our Hospitals to prioritise their use for patients with the greatest clinical need.

Transfer to Symphony Healthcare Service.

Dear Patient

Thank you for your feedback and questions following the July 2021 update regarding the transfer of the practice from Somerset NHS Foundation Trust to Symphony Healthcare Services (SHS).

We would like to thank everyone who took the time to ask questions, provide feedback and get in touch. The feedback received and questions asked were used to construct the Frequently Asked Questions document that can be found on the surgery website to support those interested in understanding more about the planned changes. Here you can find some of their frequently asked questions.

Upcoming webinars are still planned and available to all patients to provide more detail about SHS as an organisation, as well as what integration will mean. The dates of the online sessions are detailed below and to register simply click on the relevant one:

Wednesday 25 August (7pm-8pm). Presenter: Dr Harvey Sampson, Director of Strategic Development

We are now working towards the next steps and would like to confirm that we are looking to integrate into SHS in early October 2021.

While we are moving forward with the next steps of the integration, we are still open to feedback and questions and we encourage patients to continue sharing their thoughts with us.

For any questions, please do not hesitate in contacting the surgery via; [email protected] or 01984623471

All the best,

The Lister House Surgery Team
